- Alcohol License Application (must also have annual business license)
- Gaming License Application (must also have annual business license)
- Home Occupation Permit Application
- Zoning Supplemental Form (additional requirement for all commercial businesses within the city limits)
- Business License Change Form (change of location, officers, mailing address or if your business has closed)
