Project Safe & Sound

Project Safe & Sound is a notification system in which community members are able to provide first responders with information about their loved ones who experience cognitive and/or physical disabilities. The information provided is kept confidential and not published to the public. The information will only be used in the event that first responders are trying to help locate your loved ones in a time of crisis or in the event that they have gone missing.

I am a caregiver to this individual and am voluntarily providing the above information. I hereby give my permission for any first responder agency (including but not limited to Police, Fire, EMS, 911 Dispatch Center,, Search and Rescue, personnel) and medical personnel or facilities to retain and distribute the information contained in this registration form to other first responder personnel for the sole purpose of identification and/or protection of the person identified above.
